Rapid detection of carbapenemase-producing Enterobacterales (CPE) is essential for infection control. Although matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS) enables rapid bacterial identification, single-day detection of CPE directly obtained from rectal swab specimens remains challenging, because of low target abundance and competing non-CPEmicroorganisms. Therefore, we developed a selective liquid culture method and sample-preparation workflow to improve target detection. Multiple conditions such as temperature, antibiotics, oxygen availability, incubation time, and pretreatment were evaluated. The optimized workflow consisted of 4-h culture in artificial anaerobic condition using the antibiotics combination, followed by sample pretreatment and MALDI-TOF MS analysis. The resulting spectra were interpreted using the in-house CPE Typer program, which detects predefined target protein mass and assigns carbapenemase types. Preliminary evaluation using 217 clinical specimens showed high accuracy (90%). These findings suggest that our selective liquid culture combined with the CPE Typer program may support rapid carbapenemase typing from clinical specimens using MALDI-TOF MS. Larger independent cohorts are required to confirm typing performance and prediction accuracy of bacterial species |
