In subtyping adrenal incidentalomas, it is necessary to analyze various adrenocortical steroids and medullary amines using different hormone assays due to their distinct properties. To enhance bioanalytical validity and sample homogeneity, a simultaneous LC-MS analysis for twenty-nine steroids and six amines, incorporating solid-phase extraction and alkyloxycarbonylation of medullary amines was established. The assay demonstrated limits of quantification ranging from 0.1 to 0.5 ng/mL, with extraction recoveries between 58.5% and 109.5%, while precision and accuracy ranged from 3.9% to 13.9% and 89.2% to 114.9%, respectively. This validated method was applied to retrospective (n = 460) and prospective (n = 183) cohorts to characterize individual adrenal tumors, including non-functioning adenomas, pheochromocytomas, primary aldosteronism, Cushing’s syndrome, and subclinical Cushing’s syndrome. In the retrospective study, norepinephrine, normetanephrine (NMN), aldosterone, cortisol, 18-hydroxycortisol (18-OHF), 6β-hydroxycortisol (6β-OHF), 20α-dihydrocortisol (20α-DHF), and dehydroepiandrosterone sulfate (DHEA-S) were identified as tumor-specific hormones (p < 0.01 for all). The prospective study’s confusion matrices revealed that NMN, 18-OHF/20α-DHF, and 6β-OHF/DHEA-S successfully discriminated adrenal tumors with accuracies of 0.97, 0.75, and 0.80, respectively. This LC-MS-based profiling established a novel tool for subtyping adrenal tumors, improving detection through a single analysis. |
